Accountability: This position reports to the Director of Homeless and Prevention Services

Job Description:

The Building Strong Families Case Manager promotes continuity of education for children within a family in crisis. The goal of the case manager is to assist the family in the prevention of homelessness and re-establish housing stability. Core function of this position include, but are not limited to:

  • assess families referred to be admitted to the program,
  • maintain direct contact with referral sources,
  • provide direct services for parent(s) and children or Guardian/Kin and children to families referred,
  • develop a comprehensive plan with each family,
  • facilitate collaborative services needed and provide intervention counseling when necessary,
  • make full use of the resources available to JFCS and other agencies within the community,
  • perform outreach with schools and community agencies in an effort to identify families in need of the resources,
  • build relationships/partnerships through program service presentations within the community,
  • maintain relationships with the agencies in the community in order to better serve and assist clients.
